• Fosamax (Alendronate Sodium) is used in : treatment and prevention of postmenopausal osteoporosis; treatment of osteoporosis in men; treatment of Paget's disease of the bone; treatment of corticosteroid-induced osteoporosis in men and women who are receiving more than 7,5 mg of prednisone per day (or equivalent) with evidence of decreased bone mineral density.

• Fosamax inhibits resorption of bone by inhibiting osteoclast activity.Therapeutic effects of Alendronate are : reversal of the profession of osteoporosis with decreased fractures; decreased progresion of Paget's disease.

• Absorption Absorption describes the process that follows drug administration and its subsequent delivery to systemic circulation. : poorly absorbed (0,6 to 0,8%) after oral administration.
• Distribution Drug's distribution in body tissues and fluids. : transiently distributes to soft tisue, then distributes to bone.
• Metabolism and excretion Important in determining dosage regimens and intervals for persons with hepatic or impaired renal function. : excreted in urine.
• Half life The half-life of a drug is the amount of time it takes the drug level to decrease by 50% and roughly correlates with the duration of action. : 10 years (reflects release of drug form skeleton).

• Fosamax (Alendronate) is strongly contraindicated in renal insufficiency and pregnancy or lactation.
• Fosamax should be taken cautiously by : persons with active gastro-intestinal pathology (dysphagia, esophageal disease, gastritis, duodenitis, ulcers); or persons which have pre-existing hypocalcemia or vitamin D deficiency.

• Fosamax side effects and adverse reactions can be : headache, blurred vision, conjunctivitis, eye pain, inflammation, abdominal distention, abdominal pain, acid regurgitation, constipation, diarrhea, dyspepsia, dysphagia, esophageal ulcer, flatulence, gastritis, nausea, taste perversion, vomiting, erythema, photosensitvity, rash, muscoskeletal pain.

Oral administration (adults) :

• Oral administration : Administer Fosamax first thing in the morning with 6 or 8 oz plain water 30 minutes before medications, beverages or food.

• You should know the importance of taking Fosamax (Alendronate) exactly as directed, first thing in the morning, 30 minutes before other medications, beverages or food.Waiting longer than 30 minutes will improve absorption.Fosamax should be taken with 6-8 oz plain water (mineral water, orange juice, coffee and other beverages decrease absorption).If a dose of Fosamax is missed, skip dose and resume the next morning; do not double doses or take later in the day.If a weekly dose is missed, take the morning after remembered and resume the following week on the chosen day.Don't take 2 tablets of Fosamax on the same day.Do not discontinue without consulting health care professional.
• Remain upright for 30 minutes following dose to facilitate passage to stomach and minimize risk of esophageal irritation.Discontinue Fosamax and notify health care provider if pain or difficulty swallowing, retrosternal pain or new/worsening heartburn occur.
• Eat a balanced diet and consult health care about the need for supplemental calcium and vitamin D.
• Participate in regular exercise and modify behaviors that increase the risk of osteoporosis (stop smoking, reduce alcohol consumption).
• Use suncreen and protective clothing to prevent photosensitivity reactions.
• Notify health care professional if blurred vision, eye pain or inflammation occur.
• Female patient - notify health care professional if pregnancy is planned or suspected or if you're breastfeeding.
