Ultram (Tramadol) is contraindicated in : hypersensitivity, persons who are acutely intoxicated with alcohol, sedative or hypnotics, centrally acting analgesics, opioid analgesics, pyschotropic agents.Ultram is not recommended for use during pregnancy or lactation.

Use Ultram cautiously in : geriatric persons, persons with a history of epilepsy or risk factors for seizures, renal impairment, hepatic impairment, persons receiving MAO inhibitors or central nervous system depressant, increased intracranial pressure or head trauma, acute abdomen, persons with a history of opioid dependence or who have recently received large doses of opioids, children under 16 years.

Ultram side effects and adverse reactions can be : dizziness, headache, somnolence, vertigo, seizures, anxiety, asthenia, central nervous system stimulation, confusion, coordination disturbance, euphoria, malaise, nervousness, sleep disorder, vasodilation, visual disturbances, constipation, nausea, vomiting, abdominal pain, anorexia, diarrhea, dry mouth, dyspepsia, flatulence, menopausal synmptoms, proteinuria, urinary frequency, urine retention, hypertonia, respiratory depression, diaphoresis, pruritus, rash.

Ultram dosage : |
Oral administration (adults) :
- Rapid titration : 50 - 100 mg every 4 - 6 hours (don't exceed 400 mg / day or 300 mg in persons over 75 years).
- Gradual titration : 25 mg of Ultram per day initially, increase by 25 mg / day every 3 days to 100 mg / day, then increase by 50 mg / day every 3 days up to 200 mg / day.

Ultram renal impairment : |
Oral administration (adults) :
- CCr < 30 ml / min : increase dosing to every 12 hours (don't exceed 200 mg / day).

Ultram hepatic impairment : |
Oral administration (adults) :
- 50 mg of Ultram every 12 hours.
|
|
How to administer Ultram : |
- Ultram (Tramadol) may be administered without regard to meals.
- Ultram is considered to provide more analgesia than codeine 60 mg but less than combined aspirin 650 mg / codeine 60 mg for acute postoperative pain.
- For chronic pain, daily doses of 250 mg of Ultram provide pain relief similar to that of 5 doses / day of acetaminophen 300 mg / codeine 300 mg, or 2 - 3 doses / day of acetaminophen 500 mg / oxycodone 5 mg.
- Ultram should be discontinued gradually after long-term use to prevent withdrawal symptoms.
- Regularly administered doses may be more effective than PRN administration.Analgesic is more effective if taken before pain becomes severe.

Ultram may cause dizziness and drowsiness.Avoid driving or other activities requiring alertness until response to Ultram is known.

Change positions slowly to minimize orthostatic hypotension.

Avoid concurrent use of alcohol or other central nervous system depressants with Ultram.

Turn, cough and breathe deeply every 2 hours to prevent atelectasis.

Don't stop Ultram therapy abruptly.

Report any symptoms of central nervous system changes or allergic reactions.
