Zantac® (Ranitidine) patient handout

Zantac available forms :
Granules (effervescent) : 150 mg
Infusion : 1 mg/ml in 50-ml containers
Injection : 25 mg/ml
Syrup : 15 mg/ml (liquid contains alcohol)
Tablets : 75 mg, 150 mg, 300 mg
Tablets (dispersible) : 150 mg - available only in Australia
Tablets (effervescent) : 25 mg, 150 mg
Zantac indications :
  • Zantac, Zantac-C, Zantac 75, Zantac EFFERdose Tablets, Zantac 150 GELdose, Zantac 300, Zantac 300 GELdose are used in : short-term treatment of active duodenal ulcers and benign gastric ulcers; prophylaxis of duodenal ulcers (at lower doses); management of GERD; treatment and prevention of heartburn, acid indigestion and sour stomach (OTC use); management of gastric hypersecretory states (Zollinger-Ellison syndrome).
Zantac action :
  • Zantac (generic name : Ranitidine) inhibits the action of histamine at the H2 receptor site located primarily in gastric parietal cells, resulting in inhibition of gastric acid secretion.Therapeutic effects of Zantac : Healing and prevention of ulcers; decreased symptoms of gastroesophageal reflux; decreased secretion of gastric acid;
Pharmacokinetics for Zantac :
Zantac contraindications and precautions :
  • Zantac (Ranitidine) is strongly contraindicated in : hypersensitivity, cross-sensitivity may occur; some products contain alcohol and should be avoided by persons with known intolerance; some products contain aspartame and should be avoided by people with phenylketonuria.
  • Use Zantac cautiously in : renal impairment, geriatric persons (more susceptible to adverse central nervous system reactions; dosage reduction recommended); pregnancy or lactation.
Zantac adverse reactions and side effects :
  • Zantac side effects and adverse reactions can be : confusion, dizziness, drowsiness, hallucinations, headache, altered taste, constipation, diarrhea, nausea, decreased sperm count, impotence, gynecomastia, agranulocytosis, aplastic anemia, anemia, neutropenia, thrombocytopenia, pain at intra muscular site, hypersensivity reactions, vaculitis.
Zantac dosage :
Oral administration (adults) :
  • Short-term treatment of active ulcers - 150 mg of Zantac twice daily or 300 mg once daily at bedtime.
  • Duodenal ulcer prophylaxis - 150 mg of Zantac once daily at bedtime.
  • Gastroesophageal reflux disease - 150 mg twice daily.
  • Erosive esophagitis - 150 mf of Zantac 4 times daily initially, then 150 mg twice daily as maintenance.
  • Gastric hypersecretory conditions - 150 mg of Zantac (Ranitidine) twice daily initially; up to 6g/day have been used.
  • OTC use - 75 mg when symptoms occur (up to twice daily)

Intravenous & Intramuscular (adults) :

  • 50 mg of Zantac every 6-8 hours (not to exceed 400 mg/day)
  • Continuous intrevenous infusion - 6,25 mg/hour
  • Gastric hypersecretory conditions - 1mg/kg/hr; dose of Zantac may be increased by 0,5 mg/kg/hr (not to exceed 2,5 mg/kg/hr)
Zantac renal impairment :
  • Oral administration (adults) : CCr < 50 ml/min - 150 mg of Zantac every 24 hours; may be increased to 150 mg every 12 hours or more frequently if necessary; further reductions may be necessary if there is coexistent hepatic impairment;
  • Oral administration (children 1 month - 16 years) : Treatment of gastric/duodenal ulcers - 2-4 mg/kg/day in two divided doses (up to 300 mg/day).Maintenance treatment of ulcers - 2,4 mg/kg/day as a single dose (up to 150 mg/day); GERD/erosive esophagitis - 5-10 mg/kg/day in two divided doses.
  • Intravenous (children 1 month - 16 years) : 2-4 mg/kg/day in divided doses every 6-8 hours (up to 50 mg/dose)
How to administer Zantac :

Oral administration :

  • Administer Zantac with meals or immediately afterward and at bedtime to prolong effect.
  • Doses administered once daily should be administered at bedtime to prolong effect.
  • Shake oral suspension before adminsitration.Discard unused suspension after 30 days.
  • Remove foil from Zantac Effervescent Tablets and dissolve in 6-8 oz water before drinking.

Direct Intravenous :

  • Dilute each 50 mg of Zantac (Ranitidine) in 20 ml of 0,9% NaCl or D5W for injection.
  • Rate - Administer over at least 5 minutes; rapid administration may cause hypotension and arrhythmias.

Intermittent Infusion :

  • Dilute each 50 mg in 100 ml of 0,9% NaCl or D5W; diluted solution is stable for 48 hours at room temperature; do not use solution that is dicolored or that contains precipitate.
  • Rate - Administer over 15-20 minutes.

Continuous Infusion :

  • Add Zantac Infusion to D5W for a concentration of 150 mg/250 ml (no greater than 2,5 mg/ml for Zollinger-Ellison patients).
  • Rate - Administer at a rate of 6,25 mg/hour.In persons with Zollinger-Ellison syndrome, start infusion at 1 mg/kg/hour.If gastric acid output is bigger than 10 mEg/hour or patient becomes symptomatic after 4 hours, adjust dose by 0,5 mg/kg/hour increments and remeasure gastric output.
Useful advices on Zantac :
  • Take Zantac (Ranitidine) as directed for the full course of therapy, even if feeling better.Take missed doses as soons as remembered but not if almost time for next dose.Do not double doses.
  • For people who are taking OTC preparations - Don't take the maximum dose of Zantac continuously for more than 2 weeks without consulting health care professional; notify health care professional if difficulty swallowing occurs or abdominal pain persists or if vomiting blood or bloody or tarry stools occur.
  • Smoking interferes with the action of histamine antagonists.Quit smoking or at least don't smoke after last dose of Zantac of the day.
  • May cause drowsiness or dizziness.Avoid driving or other activities requiring alertness untill response to the drug is known.
  • Avoid alcohol, products containing aspirin of NSAIDs, and foods that may cause an increase in gastrointestinal irritation.
  • Increased fluid and fiber intake and exercise may minimize constipation.
  • Report onset of black, tarry stools; fever; sore throat; diarrhea; dizziness; rash; confusion or hallucinations to health care professional promptly.
  • Take Zantac (Ranitidine) once daily at bedtime for best results.
  • Dissolve 25 mg of Zantac EFFERdose tablet in at least 5 ml of water and give with a dosing cup, medicine dropper or oral syringe.
  • Avoid black pepper, caffeine, alcohol, harsh spices, extremes in temperature of food.
